JOB TITLE: Division Infectious Disease Pharmacist
 
GENERAL SUMMARY OF DUTIES - The Division Infectious Disease Pharmacist is responsible for oversightand management of an antimicrobial stewardship program at all HCAfacilities within the division to include but not limited to:

KEY INTERACTIONS – Facility Directors of Pharmacy,Corporate ClinicalServices Group, FacilityClinical Coordinators/Pharmacists, Divisionand Facility Quality Directors, Facility Risk Management, FacilityInfection Control,Division Director of Clinical Pharmacy
 
DUTIES INCLUDE BUT ARE NOT LIMITED TO:
  • Oversees antimicrobialstewardship programfor all facilities
  • Develops core strategies for Antimicrobial Stewardship program
  • Assists in development of facilityantimicrobial stewardship teams and participate in committee meetings as needed
  • Provides supporting documentation, “white papers”or other information needed to facilitate P&T Committee preparation as needed
  • Ensures provision of education and supportat HCA facilities with regard to clinical protocoljustification, execution and implementation. Provides educationthrough HPG and HCA support via webinar, tele-conference, live or other communication.
  • Develops Key Performance Indicators and Dashboards related to Antimicrobial Utilization and Stewardship
  • Prepares Antimicrobial portion of monthlyPharmacy Key Performance Indicator report
  • Ensures appropriate and safe use of pharmacy technology related to antimicrobials including Automated Dispensing Technology, IV Pumps and Maximizing Order Entry System
  • Develops and implement divisionantibiograms to improve antibiotic utilization and selection
  • Maintains effective communication with facilitydirectors and leadership teams on Antimicrobial Stewardship opportunities
  • Assists in development of “best practice”recommendations for all areas of antimicrobial management
  • Provides expertise and input to facilities on streamlining or de-escalation of drug therapy,dose optimization, IV to PO conversion, PD and PK dosing, appropriate utilization protocolsand drug restrictions
  • Assists in preparation and implementation of facilityand COE policy and procedures as needed
  • Serves as liaison with localpharmacy schools to develop student rotations and/orresidencies in infectious disease
  • Develops and maintain antimicrobial stewardship section of division website
  • Assists in preparation and implementation of pharmacystaff competencies relatedto Antimicrobial Management
  • Facilitates creation of a Divisioneducation programalong with Division ClinicalDirector for pharmacists to promoteimproved patientcare.
  • Performs other duties as assignedby DDOP
  • Participates in Division Meetingsas requested
  • Participates in HCA Corporate Clinical AdvisoryBoard and other committees as requested

  • Knowledge – Has thorough knowledge of medications and clinicalpharmacy activities. Able to interpretliterature to provideformulary guidance and pharmacoeconomic data.

  • PC Skills – proficient in the use of Microsoft Office applications and other required operating systems such as CPCS, Order Scanning Systems, Automated Dispensing Units, Packaging Equipment, Physician OrderEntry, and ClinicalIntegration to systems
  • Financial management – appliestools and processes to successfully manage drug cost budget.Able to direct cost- effective drug therapy via clinicalprotocols.
  • Project management – Able to assess work activities and allocate resources appropriately. Able to work independently and handlestress appropriately.

Job Requirements

EDUCATION
  • Pharm.D. required
  • Residency Training with ID emphasis or Board Certification preferred
     
    EXPERIENCE
  • 3-5 years experience as a clinicalpharmacist in a hospital setting
  • Minimum 12 monthsexperience in infectious disease setting
     
    CERTIFICATE/LICENSE - Licensed pharmacist
     
    PHYSICAL DEMANDS/WORKING CONDITIONS - Work will be performed in an office environment. Minimal time in pharmacy environment. Must be able to stoop, kneel, and reach. Requires hand-eye coordination and manual dexterity sufficient to operate a telephone, keyboard,calculator, other office equipment, packagingequipment and IV equipment if needed. Work is often performed under stressfulsituations. Must have ability to verballycommunicate. Some travel required. Must be able to occasionally lift up to 25 pounds. Staff must remain flexibleand available to provide staffing assistance for any/all disaster or emergency situations.
